Purpose Caring for critically ill patients is challenging in resource-limited settings in the region, where the burden of disease and mortality from potentially treatable illnesses is higher than in resource-rich areas. Since early 2020, COVID-19 has pressed the immediate need for action on this issue. With the quick spread of this virus globally, providing adequate treatment to severe/critical patients with COVID-19 is a serious problem for many countries, and challenges of higher mortality and poor treatment in resource-limited settings continue. Barriers to delivering quality critical care in these settings include lack of epidemiologic data and context-specific evidence for medical decision-making, deficiencies in health systems and resources, and institutional obstacles to implementing life-saving interventions, including data. WHO is planning to support MOH/NCDC to increase the capacity of COVID-19 case management (Pillar 7 of WHO SPRP) in Libya by increasing National capacity in quality case management for COVID-19 at both designated hospitals. PHCs to reduce mortality and improve quality of care for hospitalised patients. As part of the preparedness and response, we propose to hire a consultant for Case Management. The individual will provide supportive supervision, assessment, on-the-job training, and technical advice for MOH/NCDC staff for activities planned under the COVID-19 grants and any other tasks assigned by the supervisor. Work to be performed Support the development of case management protocols, SOPs and guidelines adapted to the Libyan context and endorsed by national health authorities.Train physicians and auxiliary staff to provide life-saving skills, critical care training, and clinical management of COVID-19 in patients in line with developed SOPs and protocols at the hospital level.Assess the capacity of intensive care units and isolation areas and needs and mobilise life-saving medicines and supplies (in coordination with COVID-19 case management Pillar Lead) to support COVID-19 isolation centres across the country.Support in capacity building critical care and life-saving skills, particularly non-invasive respiratory care among healthcare workers treating severe and critical COVID-19 cases in COVID-19 isolation centres.Support WCO in developing clinical management and oxygen scale-up strategies based on the findings (in coordination with Biomedical Engineer at WCO Libya).Support the recruitment, training, and deployment of COVID-19 mobile medical teams in selected COVID-19 isolation centres according to national priorities, in addition to monitoring the geographical distribution and performanceTrain and support community health care workers on home care for asymptomatic patients or mild cases and IPC at the PHC level.Follow up with national health authorities on reporting on case management-related indicators regularly (including clinical and oxygen data). Any other related task as given by supervisor. Deliverables During the consultancy, the Consultant will have to: Develop and finalise the case management protocols and roll them out through capacity building activities in selected COVID-19 isolation centres.Submit a biweekly, throughout the assignment, the implementation of activities during the reporting periodSubmit a monthly technical report, throughout the assignment, on work performed and achievements vis-a-vis Section 4 deliverables.Conduct and monitor capacity building in resource-limited settings to prepare and respond to treatment of critically ill patients, including delivery of training.A comprehensive report describing the need and gaps of enhancing national capacities for ICU/critical care and clinical management in resource-limited districts of Libya. MOH will use the report to identify the gaps related to response capacities and support information-based capacity building plans.Support WCO in collecting and analysing case management-related data in Libya and contribute to WCO reports/info graphs. Specific requirements Qualifications required: Essential: First university degree (Bachelors’ level or MBBCh) in Medicine and/or Surgery is required. Desirable: Master’s degree (or higher) in Anesthesia, ICU or Emergency Medicine will be considered an asset. Qualification in Board Certification or specialized medicine (anesthesia, ICU, critical care, pulmonary medicine, emergency medicine or other relevant areas), training and education, and other related areas.. Experience required: Essential: At least two (2) years of clinical experience managing respiratory diseases, especially COVID-19 ICU care for severe and critical patients. Experience in ICU, emergency, COVID-19 isolation, airway management skills, mechanical ventilation (ventilators, CPAP, oxygen concentrators), communications skills are a must with knowledge about medical ethics. The incumbent is expected to advise MOH and help design an ICU/critical care program for healthcare professionals. Desirable: Relevant experience at the international levels, in the case management and clinical care activities/infectious diseases in an outbreak, disaster or humanitarian emergency. Working experience with WHO or UN agencies, sound knowledge in research and specialty in infectious diseases or respiratory diseases are assets. Skills / Technical skills and knowledge: Experience in clinical management, building capacity, training, and deployment for ICU/critical careProven ability to multi-task across various planning and implementation processes within a highly demanding environment.Proven understanding of the management of acute and urgent public health responses, including infectious disease outbreaksExperience in outbreak responseAbility to work as part of a team.Strong organisational skillsProficiency in Microsoft Office (Word, Excel, PowerPoint) Language requirements: Expert level of Arabic and English (Read – Write – Speak) is required. Place of assignment The Consultant is expected to perform her/his duties from Tripoli, with a frequent travel to Benghazi and Sabha.
